Supporting your health care journey
Our navigators are a team of experienced nurses who have knowledge of the hospital, community and primary health system. They will work with you, and anyone that matters to you, to achieve your health goals.
We have generalist navigators for people of all ages, and specialty navigators for:
- diabetes and endocrinology
- midwifery
- paediatrics
- respiratory
- stroke.
Who can get help
We care for community members who experience chronic and complex health issues and need support to manage their health conditions and well-being.
This may include people who have:
- multiple chronic conditions or new conditions expected to last longer than 6 months
- a high need for health services
- complex health conditions
How we can help
The navigator can:
- manage your appointments and act as a central point of contact for your care providers
- help you better understand your health by providing information that is relevant to your situation
- assist with communication between yourself, health professionals, and other community/support services
- link you to services that help meet your health goals
- work with the you to bring services together to ensure you get the right care, in the right place, at the right time.
Finding a nurse navigator
Your health care professional can refer you to a navigator. They'll ask for your consent first.
